2006-Current: 4People Case Management System has been used in Benton and Franklin Counties to case manage 1000 homeless or potentially homeless people. The results is a dramatic 27% reduction in homelessness in one year. This was done by government, non-profit, and faith based organizations coordinating services on the 4People Case Management system. The Home Base Committee of concerned citizens found that there was a 10% Housing Vacancy in our area, but the gap was having informed Caseworkers help people obtain the housing. A grant was received to use the 4People Case Management to reduce the recidivism of inmates transferring to the community. In Yakima County 8 Health Care organizations used the 4People Case Management to provide care for over 3000 individuals. Other gaps showed that shelters and services to youth were unavailable and needed to be developed, and food was only available during certain hours when at-risk families would be working or unavailable. As gaps are being identified the communities are finding ways to fill the gaps.

4People can provide the TOOL for the community to communicate, collaborate, and serve the less fortunate.
