a group of government, non-profits, faith based organizations, and concerned citizens use the 4People Case Management System to reduce homelessness and coordinate services to at-risk families and children in Benton and Franklin Counties, Washington State. As of 12/31/06 over 980 homeless or potentially homeless have been served.

Kids ConnectKids Connect a group of 8 health care clinics and organizations use the Case Management system to connect kids to health care in Yakima County, Washington State. As of 12/06 over 2100 children have been served.

Justice Centers
are exploring using 4People Case Management system to connect inmates with community resources prior to being released.

Faith Based organizations are using 4People Case Management to share resources with at risk families.

at-risk families is an involved and informed caseworker.

Client Registry
A demonstration of the Client Registry system during which users will learn the basics of how to access client records, manage client information, and document client information, and locate housing and shelter using the Client Registry and system tools.

On Line Support Assistant (OLSA)
& Shelter/Housing

This is an introduction to the 4People Client Registry where information is gathered and documented to allow organizations to record the work they are doing with a client and share information with other agencies that may also be working with the client.

Audience:
Case Managers, Service Providers, Supervisors

Approximate Duration:
1 hour

Objectives:

  • Understand how to obtain resources for clients in need, and report updates in information.
  • Understand how the 4People system assists in the coordination of services for clients.
  • Navigate the 4People site to access tools and resources.
  • Use data entry fields and tools to enter meaningful information to support the case management process and share information.
  • Learn to use the Shelter Housing tool to locate hard to find housing and support.
  • Follow data entry formats consistent with 4People policies.
  • Seek guidance on the use of  4People system.
  • Understand What Case Management Configuration is Best for You?

Skills:

  • Locate and create client records in 4People.
  • Enter Client and Case Management information in available fields consistent with purpose and data entry formats.
  • Locate and enter information regarding housing options.
  • Document referrals, services, and case management notes.
  • Use additional system tools to manage case management process.
  • Locate available resources to support use of 4People.

Follow-Up:

  • Review CW activities and contact in 2 weeks and 1 month to address any further questions.
