a group of government, non-profits, faith based organizations, and concerned citizens use the 4People Case Management System to reduce homelessness and coordinate services to at-risk families and children in Benton and Franklin Counties, Washington State. As of 12/31/06 over 980 homeless or potentially homeless have been served.

Kids ConnectKids Connect a group of 8 health care clinics and organizations use the Case Management system to connect kids to health care in Yakima County, Washington State. As of 12/06 over 2100 children have been served.

Justice Centers
are exploring using 4People Case Management system to connect inmates with community resources prior to being released.

Faith Based organizations are using 4People Case Management to share resources with at risk families.

 4People Case Management System TRAINING

Studies show that the greatest indicator of success for
at-risk families is an involved and informed caseworker.

Comprehensive Case Management/CCM and HMIS
4People facilitates training specific to completing case management tasks related to the use of 4People CCM/HMIS. All organizations receiving HUD funding are required to report using an HMIS system. Participants will learn how to navigate the 4People system to enter information gathered from HMIS Intake and Exit forms, enroll clients into programs, understand data required for the Annual Progress Report (APR) and how to avoid common errors.

Prerequisites:

  • Completed Client Registry & On Line Support Assistant (OLSA) training.
  • Knowledge of services and HMIS programs provided by your agency.
  • An agency provided email account.
  • Understand What Case Management Configuration is Best for You?

Audience:
This trainings appropriate for all case managers, including case manager supervisors that receive HUD funding or provide services to the homeless or potentially homeless.

Approximate Duration:
1 hour

Objectives:

  • The training will instruct on the differences of the General Information Tab in CCM and the OLSA Case Management.
  • The training will instruct on the difference between a service and referral.
  • How to input referrals and services provided into a client record using multiple tabs
  • Use data entry fields and tools to enter  HMIS compliant information to support the HUD APR.
  • Sharing - when to and when not.

Skills:

  • By the end  the case managers should be confident in tracking and reporting the services that they provide HMIS clients as part of the case management process.
  • Document referrals, services, and case management notes using CCM/HMIS
  • Use additional system tools to manage case management process.
  • Know when and how to share client information.

Follow-Up:

  • Review CW activities and contact in 2 weeks and 1 month to address any further questions.
